Adult Enteral Feeding Policy (P48)
The purpose of the policy is to provide best practice guidance for the care of adults fed via with an enteral feeding tube. The aim of the policy is to ensure delivery of safe and effective care for patients who are enterally tube fed within Derbyshire Community Health Services: • Community Hospitals, • Community Services • Care Homes And to ensure continuity of care within Derbyshire Community Health Service Foundation Trust.
Clinical Record Keeping Policy and Standards (P6)
This policy aims to ensure that the clinical records made by staff are fit for purpose and of a quality that provide for objective, accurate, current and comprehensive information that supports and enables the best clinical care and treatment for the patient/client. This policy has incorporated a range of best practice and related legislative requirements to outline the organisations expectations for clinical record keeping standards, both on paper and electronically. The policy provides support to the organisation in meeting its statutory and legal obligations as laid down by the Records Management: NHS Code of Practice 2016; Data Protection Act 1998 section 7, General Data Protection Regulation 2018 and relevant professional bodies. The policy also identifies the standards expected of all registered and non-registered staff. It sets a minimum standard, which will be applicable to all patient settings, including community clinics and inpatient areas. This policy does not replace standards set by professional organisations, but is complementary to them and should be used in conjunction with them.
DCHS Safeguarding Supervision Policy (P15)
Safeguarding supervision is recognised by DCHS FT as an important element within clinical supervision and the safety culture. In addition to an individual’s knowledge, skills, experience and training, effective safeguarding practice relies on a professional’s curiosity and vigilance. Safeguarding supervision provides an opportunity to both sustain and foster these qualities and ensure staff are updated on current safeguarding issues, legislation and outcomes of recent Child Practice Reviews (CPR), Serious Incident Learning Reviews (SILR), Domestic Homicide Reviews (DHR), and Safeguarding Adult Reviews (SAR). Health Visitor to School Nurse Transfer Guidelines (G250)
This guidance document outlines the minimum standard expected of Specialist Community Public Health Nurses - School Nurses and Health Visitors - when transferring records prior to a child commencing in full time education. It outlines the goals of the transfer process and the roles that each professional concerned plays within that process. This document supports a commitment to evidence-based care. It is expected that Specialist Community Public Health Nurses employed by DCHS exercise professional judgement and have clear, documented evidence of the rationale for their decision making if they feel that it is not in the family’s or child’s interest to follow the SOP or policy has not been followed. It is also expected that Specialist Community Public Health Nurses exercise sound professional judgement (following their assessment) in deciding whether a client/family should receive additional support and intervention. This Standard Operating Procedure should be used in conjunction with other DCHS Childrens and DCHS Trust policies along with The NMC Code.
