Digital ReSPECT to be rolled out on Derbyshire Shared Care Record

From June 2025, digital ReSPECT (Recommended Summary Plan for Emergency Care and Treatment) for Derbyshire patients will be created and stored on the Derbyshire Shared Care Record (DSCR).

This allows multiple organisations to contribute to a single ReSPECT plan – editing and updating it as needed – while reducing the risk of patients having multiple active plans and preventing important information about their choices regarding their care being missed.

A ReSPECT plan creates personalised recommendations for a person’s clinical care and treatment in a future emergency in which they are unable to make or express choices. These recommendations are created through quality conversations between the person, their families, and their healthcare professionals, to understand what matters to them and what is realistic in terms of their care and treatment.  

By embedding digital ReSPECT plans into the DSCR, all health and care professionals, including emergency-out-of-hours, ambulance, GPs, community services, care homes, and hospital teams, will have real-time access to information about the patient including their personal preferences and priorities, and what is realistic in terms of their care and treatment. 

This enhancement reinforces the DSCR as the single source of truth for shared care information for healthcare colleagues across Derbyshire, enabling coordination across the entire Joined Up Care Derbyshire healthcare system. 

* Please note: ReSPECT plans remain a patient held paper document. Black and white ReSPECT plans are valid, and digital ReSPECT plans don't need a handwritten counter signature.

Digital ReSPECT plans will be rolled out across the DSCR from June onwards. This will be a soft launch and you will still be able to use your existing way of creating ReSPECT plans initially (ie creating them electronically within your own clinical system, or handwriting plans). 

At some point in the future, other means of creating electronic plans will be switched off, with the expectation that all ReSPECT plans will be stored digitally on the DSCR. There will always been the need for handwritten ReSPECT plans for use in exceptional circumstances, but it is anticipated that these will be transcribed across to the DSCR as soon as clinically possible.  

We will communicate to you when all digital ReSPECT forms need to be stored in the DSCR in the coming months. 

Digital ReSPECT plans will be available to colleagues organisations across Derby and Derbyshire from June onwards:  

  • From June: University Hospitals of Derby and Burton  
  • Launch date to be confirmed: Chesterfield Royal Hospital, Derbyshire Healthcare Foundation Trust; General Practice, Derbyshire Community Health Services, DHU Healthcare, Hospices, East Midlands Ambulance Service 

Weekly drop-in training and Q&A sessions on how to use and access the DSCR and its functionality, as well as an overview of the digital ReSPECT plans, will take place from 12.30pm until 1.30pm every Tuesday, Wednesday and Thursday via MS Teams. Join the DSCR drop-in training sessions on MS Teams here.
