Implementation of National Guidance Policy (P44)
All patients should have fair access to high quality care which is based on clear evidence of best practice. There are many examples of documents issued either by the Department of Health or bodies such as the National Institute of Health and Care Excellence (NICE) which set out the requirements for organisations to follow either as mandatory targets or as best practice guidance and professional advice. This policy sets out the process for the dissemination and implementation of national guidance within Derbyshire Community Health Services NHS Foundation Trust (DCHSFT). This policy aims to provide a clear process to ensure that national guidance for example NICE, Care Quality Commission Reviews, or NHS Improvement, are appropriately disseminated implemented and monitored across the organisation.
Dressing Formulary and Wound Care Guidelines (G68)
The Derbyshire Wound Care and dressing formulary has been revised in collaboration with the East Midlands Wound Care Formulary Group. Work has been undertaken to provide a clinically effective, appropriate and cost effective choices of products to manage the vast majority of wounds. The formulary is available for all practitioners working for Derbyshire Community Health Services and Primary Care Services. It is expected that prescribers will preferentially use the products listed in the guide for routine use and be able to provide robust rationale where they have prescribed outside the formulary.

Standard Operating Procedure (SOP) for The T34 and Bodyguard Syringe Drivers (S61)
A subcutaneous infusion pump is an effective method of administering therapeutic drugs via subcutaneous route, which can no longer be tolerated orally or rectally by the patient. The aim of this SOP is to provide guidance for staff in the use of the McKinley T34 pump across adult services. Using a uniform SOP will reduce the level of risks associated with syringe pump management within DCHS.

This policy outlines the Trust’s approach for the prevention of pressure ulcers in people under the care of DCHS clinicians. It encompasses the appropriate management for the prevention of pressure ulcers as well as the management of patients with ulcers already present. This policy and the supporting guidelines place an emphasis on a collaborative integrated multidisciplinary, multiagency approach to identifying risk factors and the implementation of appropriate preventative and/or treatment measures in a timely manner.

Conservative Sharp Debridement (CSD) of Wounds Policy (P99)
We know that wound dressings should provide the optimal environment for wound healing after careful clinical assessment of the wound, the patient clinical condition and clinical outcomes. Hard to heal wounds, however, fail to progress, and many can be seen with dead tissue, slough and necrosis which in turn can increase the risk of infection and also increase cost within appropriate dressing use and nursing time. This procedure will only be undertaken by the Tissue Viability team members who have successfully completed a validated programme of care and assessment of competence in practice. A minimum of 10 episodes of observed practice is to be completed until competency can be granted. The purpose of debridement describes any method by which such materials (necrosis, slough and dead de-vascularised tissue) are removed and consequently the potential to achieve wound healing enhanced. It also reduces the bacterial load and minimises the risk of local and systemic infection. It can allow for wound draining and reduce odour overall enhancing patient quality of life.
