The Trust’s aim is to prevent harm resulting from falls that may occur by assessing each patient and identifying their individual risk and the interventions required. There is an expectation that clinicians who work in the community and who see patients in their own homes, extended care settings or in outpatient settings will use the policy framework as part of their everyday practice within DCHS. Inpatient Falls Prevention and Management Policy (P33)
The Trust’s aim is to prevent harm resulting from in-patient falls by assessing each patient individually and identifying their risk in order to develop a care plan to reduce these risks. There is an expectation that clinicians will use the policy framework within everyday practice within DCHS. DCHS Inpatient services are part of the DCHS Falls and Fracture Prevention framework which contributes towards the wider Derbyshire and Derby City Falls and Fracture Prevention Pathway in identifying and managing patients who are at risk of falls in hospital and onwards as part of discharge plans and communication. This policy incorporates guidance from the: - • NICE CG161(2013) “Falls: the assessment and prevention of falls in older people • National Patient Safety Agency (2011) on “Essential care after an inpatient fall” • NICE (2015) on “Head Injury: assessment and early management” The purpose of this policy is to support staff to identify patients who are at risk of falling and to identify the interventions required to reduce the risk of falls and minimise harm to each individual. This policy also provides guidance on how to implement the NICE Quality standards (2015) ‘Assessment after a fall and preventing further falls’
Risk Management Policy
The aim of this document is to provide clear and accurate direction & guidance to risk management for all staff within Derbyshire Community Health Service NHS Foundation Trust (DCHS). Risk management is the recognition and effective administration of all threats that may negatively impact upon values, standards & reputation of DCHS thus preventing planned objectives that in turn may preclude the Trust in its delivery of high quality statutory responsibilities. Risk management also includes positive exploitation of any opportunity that may present during threat analysis or mitigation. The purpose of this policy is to evidence the importance of risk management to DCHS, maintain a consistent approach to effective risk management, ensure accurate & effective systems and processes are firmly in place to support all staff in the management of corporate and operational risks across the organisation. Provide a single point of reference for information pertaining to all contributing facets, platforms, staff & agencies involved in the management of risk throughout all areas of service provision. DCHS’ risk Management policy seeks to mitigate risks that may threaten delivery of planned strategic objectives and put in place measured controls to manage such risks to as low as reasonably practicable.
