Tier 3 Weight Management - Information for Patients

Here you will find all the information you will need if you have been referred to our service. It will also help those who would like to know more about how our service works.

You can be referred to our service if:

  • You have a BMI of over 50.
  • You have a BMI of 35-49, with other health problems.  

You can find out your BMI by using the NHS BMI calculator.

You cannot self-refer to our service. You can be referred by:

  • Your GP.
  • A practice nurse.
  • Any consultant doctors you see in the hospital. 
  • A clinical pharmacist. 

To get the most out of our service you must be ready to change and engage with us. There are two different parts to our new service plan.

Our new programme starts with groups and online support 

We have joined forces with services that offer support and education so that you can learn the latest weight loss information and guidance.  

Anyone opting in will be invited to a 12-week group education and support programme (online or face to face) for the first time or as a refresher to:  

  • Learn healthy habits  
  • Get emotional and lifestyle support  
  • Make changes that improve your health  
  • Plan your next steps

If you live in Derby City: this will be Livewell Derby.

If you live in Derbyshire: this will be Live Life Better Derbyshire.

If you have diabetes, you will also have the option to complete the NHS Weight Managment Programme.

Clinical reviews for individuals  

For those with a high BMI and who require additional support a second stage of input will be available when you have attended and completed the group programme. You may receive up to three appointments with members of our clinical team or our weight management advisers and will also have access to further information and support to match your health needs over 3 months.

Tier 2 Weight Management Services are for people with a BMI of 25 or above.  You can self-refer to Tier 2 Weight Management. 

If you have been referred to us:

  • We will review the information sent to us & make sure that you have completed the foundation skills via the group education sessions.
  • Once eligible we will arrange for you to speak with a weight advisor to get started on some ideas to help you structure your eating and exercise habits.
  • You may have additional appointments with a psychologist, dietician or medical doctor if we think that this would be helpful and especially if you are hoping to be considered for bariatric surgery.
  • We offer phone, web based (Microsoft Teams) or face to face appointments.

If you are thinking about weight loss surgery in the future, you will need to still complete the 12 weeks education aspect of our programme to prepare you with what dietary changes you will need to ideally maintain pre and post-surgery. You will be assessed by a specialist dietician and a doctor as well as one of the psychologists, to ensure that you are in a good place to consider surgery. Surgery can be a big thing, so we want to make sure you are mentally and physically prepared.   

Weight loss surgery is only successful in the long term when you build in good habits in your eating and exercise habits.   

Once all members of the team agree, you may then be referred on to a tier 4 surgical service.
