Trust Policy for Free of Charge (FOC) Medicines Schemes (P109)

This policy is to ensure consistent, equitable and robust consideration of any free of charge (FOC) scheme that DCHS clinicians would like to be considered for use within DCHS, in line with the National guidance on FOC schemes. Including the requirement for consideration of schemes within Derbyshire for system approval prior to use within DCHS by the Derbyshire Joint Area Prescribing Committee. Where the medicine is currently commissioned by NHS England Specialised Commissioning, ICSs should discuss all FOC schemes with their regional specialised commissioning team in the first instance, before a decision is made on whether to implement the FOC scheme.

Promotion and Management of Continence for Adult Services Policy (P10)

This policy aims to identify a framework for the standards of care and best practice for bladder, bowel and continence promotion. The range of multidisciplinary professionals involved in continence care is diverse, and it is therefore essential that a continence service delivers integrated working practices across organisational and professional boundaries in order to provide effective care and efficient use of resource. The information detailed within this document will assist healthcare professionals who are undertaking a continence assessment and sets the standards of care for patients who present with a bladder or bowel problem. The continence advisory service aims to provide a quality service to all adults registered with a Derbyshire or Derby City GP. People with continence needs should be seen at the most appropriate time by the most appropriate professional. Excellence in continence care (2018) suggests that the initial assessment is best undertaken by staff trained in continence care within in a community setting, the provision of a high-quality assessment is the foundation of high-quality continence care.

Dressing Formulary and Wound Care Guidelines (G68)

The Derbyshire Wound Care and dressing formulary has been revised in collaboration with the East Midlands Wound Care Formulary Group. Work has been undertaken to provide a clinically effective, appropriate and cost effective choices of products to manage the vast majority of wounds. The formulary is available for all practitioners working for Derbyshire Community Health Services and Primary Care Services. It is expected that prescribers will preferentially use the products listed in the guide for routine use and be able to provide robust rationale where they have prescribed outside the formulary.
