Missing Patient Procedure Including Section 18 Concern for Safety and Welfare (S23)
This procedure has been developed to inform hospital staff what action to take when a detained patient is absent without leave from the hospital where they are liable to be detained under the Mental Health Act 1983. Guidance when an informal patient’s whereabouts is unknown and this includes patients that may be missing from other in-patient areas. The procedure aims to provide a consistent and easy to follow approach thus supporting the staff and protecting some of our most at risk and vulnerable patients.
Requesting and Managing Pathology Results within DCHS Community Hospital Wards SOP (S98)
The purpose of this Standard Operating Procedure (SOP) is to outline the steps required to effectively manage the requesting, receiving, filing and actioning of all pathology results by either an electronic process or by a relevant paper-based system. Utilising an electronic system (such as ICE) enables pathology requests to be requested, reviewed and actioned electronically via the electronic patient record within TPP SystmOne. There are an estimated 1.12 billion pathology tests undertaken each year in England (NHS England, 2020) It is imperative a record of all pathology samples is accurately maintained to avoid patient harm and improve patient outcomes (WHO, 2021). The Care Quality Commission (2021) inspects the management of test results to ensure processes are robust, practice is safe and care is effective.

The purpose of this document is to summarise the procedures that staff should follow when transmitting patient and personal information. Other DCHS NHS Trust policies contain more detailed information on the responsibilities of staff in relation to confidentiality and information security, and therefore all staff should ensure they have read and understood their full responsibilities in these areas.
Delivering Same Sex Accommodation (P64)
Every patient has the right to receive high quality care that is safe, effective and respects their Privacy and Dignity. There are no exemptions from the need to provide high standards of privacy and dignity and this applies to all areas, including when admission is unplanned. This is one of the guiding principles of the NHS Constitution (2009) and at the core of local NHS visions. Derbyshire Community Health Trust ( DCHS) aim is that all patients who are admitted to any of our hospitals will only share the room where they sleep with members of the same sex, and same sex toilets and bathrooms will be close to their bed area. Sharing with members of the opposite sex should only happen by exception based on clinical need (for example where patients need specialist equipment), or when patients choose to share (for instance married couple who have been admitted together may want to share a side room). This Policy contributes to the achievement of CQC Outcome 4 – The patient will receive care, treatment and support in single sex accommodation wherever it is available. The aim is to ensure a clear and consistent approach is adopted across DCHS community hospitals by all ward managers.
Procedure for the Disposal of unwanted patient’s medicines in the community setting (patient’s own home) (S7)
Guidance to community staff on the procedure to follow regarding the destruction of a patient’s own medicines that are no longer required and are within the home setting.
Standard Operating Procedure (SOP) for The T34 and Bodyguard Syringe Drivers (S61)
A subcutaneous infusion pump is an effective method of administering therapeutic drugs via subcutaneous route, which can no longer be tolerated orally or rectally by the patient. The aim of this SOP is to provide guidance for staff in the use of the McKinley T34 pump across adult services. Using a uniform SOP will reduce the level of risks associated with syringe pump management within DCHS.
Deprivation of Liberty DoLs Policy (P35)
The Mental Capacity Act 2005 aims to empower people to make decisions themselves wherever possible and sets out the steps which must be taken to promote this. Where a person lacks the capacity to make a particular decision it provides a statutory framework for acting and making decisions on their behalf, and in their best interests (see the DCHS Mental Capacity Act Policy and Appendix 3 Tips on assessing capacity) The Deprivation of Liberty Safeguards (DoLS) is an addendum to the Mental Capacity Act (2005) that came into force in 2009. It ensures that any Best Interests decision that deprives someone of their Article 5 right to liberty (European Convention of Human Rights) is made according to defined processes and in consultation with specific authorities. It applies where a person needs to be accommodated in a hospital or a care home in order to receive care or treatment for which they cannot consent. The DoLS were introduced to protect an individual’s rights under such circumstances, and ensure that any care or treatment that they receive, including where this involves the use of restraint or restrictions, is proportionate to the risk of harm they would otherwise be at and in their best interests.
