Falls Management Policy for use in Urgent Treatment Centres, Community and Outpatient settings (P32)

The Trust’s aim is to prevent harm resulting from falls that may occur by assessing each patient and identifying their individual risk and the interventions required. There is an expectation that clinicians who work in the community and who see patients in their own homes, extended care settings or in outpatient settings will use the policy framework as part of their everyday practice within DCHS. This policy incorporates key national guidance: - • NICE CG161 (2013) “Falls: the assessment and prevention of falls in older people • NICE Quality standards (2015) ‘Assessment after a fall and preventing further falls’. • NICE (2015) on ‘Head Injury: assessment and early management’ • BGS Fit for Frailty (2014) ‘Consensus best practice guidance for the care of older people living in community and outpatient settings’ • Public Health England (2019) ‘Preventing falls in people with learning disabilities: making reasonable adjustments’

Inpatient Falls Prevention and Management Policy (P33)

The Trust’s aim is to prevent harm resulting from in-patient falls by assessing each patient individually and identifying their risk in order to develop a care plan to reduce these risks. There is an expectation that clinicians will use the policy framework within everyday practice within DCHS. DCHS Inpatient services are part of the DCHS Falls and Fracture Prevention framework which contributes towards the wider Derbyshire and Derby City Falls and Fracture Prevention Pathway in identifying and managing patients who are at risk of falls in hospital and onwards as part of discharge plans and communication. This policy incorporates guidance from the: - • NICE CG161(2013) “Falls: the assessment and prevention of falls in older people • National Patient Safety Agency (2011) on “Essential care after an inpatient fall” • NICE (2015) on “Head Injury: assessment and early management” The purpose of this policy is to support staff to identify patients who are at risk of falling and to identify the interventions required to reduce the risk of falls and minimise harm to each individual. This policy also provides guidance on how to implement the NICE Quality standards (2015) ‘Assessment after a fall and preventing further falls’

This standard operating procedure sets out the process by which clinicians working within Integrated Community Services are able to utilise a seated lifting device (e.g. Mangar Elk or Camel) to support patients who have fallen in their home environment. This will prevent long lies and use of emergency ambulance personnel for retrieval only. The aim of this procedure is to support clinicians to: • Understand the purpose and use of the device • Ensure the response using the device is appropriate. • Ensure clinical documentation is properly completed.
