Tier 3 Specialist Weight Management Service Operational SOP (S157)
The guidance defines what the patient journey through the Tier 3 Specialist Weight Management Service should look like, including: • How referrals are received • How referrals out of the service are completed (to Tier 2 and Tier 4 services) • Clinical thresholds and triage criteria • Patient flow and program content • Prescribing initiation and transition points • MDT roles and responsibilities • Discharge process It also aims to ensure equitable access, clinical safety and consistency in triage and discharge decisions across localities.

This policy details the procedures the Trust has adopted for dealing with applications for access to personal records held by Derbyshire Community Health Services, whether formal manual (for example, on paper) or computerised (such as within a clinical system) or information which could be classed as informal which may have been written within emails, text or recorded in Microsoft Teams, for example.
Domestic Abuse and Sexual Violence Policy (P90)
To provide information about the practical help and support that is available to employees of DCHSFT who are currently experiencing domestic abuse, or are experiencing trauma as a result of their past experiences of abuse. This Policy also covers the approach to be taken where an employee’s behaviour towards their family may constitute domestic abuse. Providing this information empowers Managers to take effective, supportive action and reinforces the message that domestic abuse and sexual violence cannot be ignored.

Development, Approval, Implementation and Review of Patient Group Directions (PGDS) Policy (P5)
The purpose of this policy is to set out a generic framework for a co-ordinated approach to the development and control of PGDs in use in Derbyshire Community Health Services (DCHS) Trust. The policy contains a standard template for all locally developed PGDs. Using the framework and template should ensure that PGDs comply with the legislation and are reviewed and updated every 3 years or in response to updated guidance, stock availability, safety alerts or best practice.
DCHS Safeguarding Supervision Policy (P15)
Safeguarding supervision is recognised by DCHS FT as an important element within clinical supervision and the safety culture. In addition to an individual’s knowledge, skills, experience and training, effective safeguarding practice relies on a professional’s curiosity and vigilance. Safeguarding supervision provides an opportunity to both sustain and foster these qualities and ensure staff are updated on current safeguarding issues, legislation and outcomes of recent Child Practice Reviews (CPR), Serious Incident Learning Reviews (SILR), Domestic Homicide Reviews (DHR), and Safeguarding Adult Reviews (SAR).
