We provide two services for people with diabetes - Community Diabetes for people with Type 1 & Type 2 diabetes and our Diabetes Education service for people with Type 2 diabetes in Derby and Derbyshire. Each service is designed and selected according to individual requirements.

Learning from Death’s Policy (P72)
This policy confirms the process to ensure a multi-disciplinary, consistent and coordinated approach for the review of deaths that occur in all DCHS in-patient and community team caseloads. The aim of the learning from deaths process is to identify any areas of practice both specific to the individual case and beyond that could potentially be improved, based upon peer group review. Areas of good practice are also identified and supported. To describe in detail the three-stage mortality review process within the Trust, detailing how reviews should be completed, by whom and when to ensure that learning from deaths is made a Trust priority and leads to developments and improvements in patient care.

Mental Health Act 1983 - Detention of Informal Patients under Section 5(4) Policy and Procedure (P104)
This policy provides guidance on the use of Section 5(4), nurses holding power under the Mental Health Act 1983. It should be followed by all Registered Mental Health Nurses and Learning Disability Nurses working in trust in-patient areas. This short-term power, not more than six hours, can be applied to informal patients under certain conditions as below. This policy should be read in conjunction with Chapter 18 ‘Holding powers’ of the Mental Health Act 1983 Code of Practice (2015).
