Information About Your Appointment

We offer our services from a range of locations across Derbyshire. If you are not able to find a suitable appointment at your closest site, you can choose to attend your appointment at any of our other sites.

To improve the quality of your care, we recommend that you stay with the same healthcare professional for your whole course of treatment. Please consider this when choosing the location of your first appointment

Decision Support Tools

It may also be helpful to look at one of the NHS Decision Support Tools. These are available for certain conditions and are there to support you to make an informed decision about your treatment. For more information, please watch the video below.

Produced by Keele University Impact Accelerator Unit and Midlands Partnership University NHS Foundation Trust, in partnership with and funded by the NHS England Personalised Care Team in the Midlands region.

Virtual Consultations

We offer the choice of face-to-face and virtual consultations to everyone accessing our service.

Virtual consultations have become increasingly popular as a safe and convenient alternative to face-to-face appointments.

A virtual consultation can be carried out by telephone or video call, depending on what your problem is and your preferences. Our healthcare professionals are experienced in carrying out appointments in this way and will be able to complete a thorough assessment, explain what could be causing your problem, and support you to decide on the best plan for you.

Your healthcare professional can arrange a face-to-face appointment for you if needed after your telephone or video call.

For information about how to join a video call, please read: DCHS Guidance for Video Consultations.

Non Emergency Patient Transport Service (NEPTS)

This service is for eligible patients who need medical or clinical support to get to and from their healthcare appointments. Click on the link above for more information.

Travel Derbyshire on Demand

This is a new type of bus service for everyone in the Districts of Bolsover, North East Derbyshire and Chesterfield Borough.

It can be booked for any type of journey in these areas, including getting to a medical appointment. Click on the link above for more information.

After your appointment you may be offered a PIFU. This will be decided after a discussion between you and your healthcare professional.

PIFU is a type of open appointment that gives you or your carer the flexibility to arrange your own follow up appointment whenever you need it, during a 12-week period from your last appointment.

If your symptoms flare up or your circumstances change within this period, you can easily book a follow up appointment with your clinician. Contact us via telephone or online to book.

Following the 12-week period, if we have not heard from you, you will be discharged from our service. Please note if you have been discharged from the Musculoskeletal Therapy service, you can self-refer back into the service at any time.

Click here for information about self-referrals.

We regularly review the measures we have in place to reduce the risk of infection and protect patients, staff, and visitors. Please note that you may be asked to wear a face mask when visiting our sites unless exempt for medical reasons. For the most up-to-date information, please see posters around our sites or ask a member of staff.

If you are unwell or have symptoms of Covid-19 when your appointment is due, please contact us via telephone. We will offer you the choice of a telephone appointment or rearrange your appointment.
