P24 Care Programme Approach Policy
This document summarises the responsibilities placed on health and social Services under the Care Programme Approach (CPA). CPA is designed to give a systematic approach to care for all service users particularly those with complex needs or at high risk of hospital admission.
These objectives apply to all patients receiving treatment and care from the Older Peoples Mental Health and Learning Disabilities Services, whether or not they are admitted to hospital.
