About our Service
Who are ESSD?
- We are the Early Supported Stroke Discharge (ESSD) Service. We are a Specialist Stroke Team consisting of Therapists, Nurses, Therapy Assistants and Admin staff.
What do ESSD do?
- We work with patients who have had a Stroke, visiting them regularly in their own homes soon after discharge from hospital. We tailor our therapy and support to the specific lives and goals of our patients, helping patients and their families to get back to life after stroke.
- We can work with patients for up to 6 weeks and then will transfer on to the most appropriate service if further therapy and/or support is required. We have a specific criteria meaning that not all patients who have had a stroke will come to our service.
Where do ESSD work?
We see patients in their own homes across South Derbyshire including:
- Derby City, South Derbyshire Dales and Amber Valley and Erewash.
What do we expect from patients?
- For patients to make the best recovery after stroke we will design and recommend exercises, techniques and activities to be practised throughout the day in between our therapy sessions – it’s a team effort!
Your hospital team will let you know if you are being referred to our ESSD Service on discharge home from hospital.
We will contact you to arrange our first visit to you at home.
