About our Service
We provide specialist assessment and rehabilitation to patients with Stroke and Neurological conditions across Chesterfield and North East Derbyshire for people registered with a Derbyshire GP.
Our Physiotherapists, Occupational Therapists, Therapy Assistants and admin support workers work together to deliver high quality services for adults diagnosed with a neurological condition.
We have clinics based at four localities in Chesterfield, Dronfield, Clay Cross and Clowne. We can also provide home visits for those who meet service criteria.
What happens now? When your referral has been received it will then be triaged to determine if it’s appropriate. If it is accepted, you will be placed on our waiting list and sent a letter to confirm. We will then contact you to arrange an appointment. This may be over the telephone or face to face.
What to bring to an appointment? Please bring an up to date medication list and any recent clinic letters related your neurological condition
Can I bring a family member? Patient’s will be welcomed to bring a family member, carer or friend to the appointment.
How can I get to my appointment? There are free car parks at all of our sites including disabled bays. If eligible you can access Patient Transport or Travel Derbyshire on demand.
What happens if I am unable to make my appointment? Please contact our service at the earliest opportunity to let us know and re-book if appropriate.
