About our Service:

We provide high quality specialist assessment and rehabilitation to adults diagnosed with Stroke and Neurological conditions across Amber Valley and Erewash, who are registered with a Derbyshire GP.

Our Neurological Outpatient team consists of Physiotherapists, Occupational Therapists and Support Workers.  We offer clinics at Ripley and Ilkeston Community Hospitals.

We aim to help promote your independence in your daily activities and to support your quality of life by offering advice and education to you and your family. We will support you to self-manage your condition with appropriate strategies.

What happens now? When your referral has been received it will be triaged to determine if it’s appropriate. If it is accepted, you will be placed on our waiting list and sent a letter to confirm.

Once we have availability we will contact you to arrange an appointment. This may be over the telephone or face to face.

Support while you wait: 

While waiting for your appointment, we encourage you to explore the resources on our website : Stroke and neurological services : Derbyshire Community Health Services It has useful information to help you manage your condition, including advice and exercises.. It also provides information about when it may be appropriate to seek further medical advice. 

What to bring to an appointment? Please bring an up to date medication list and any recent clinic letters related your neurological condition.

It would be helpful if for your first appointment you can think about the things you would like to address in your therapy sessions.

What do I need to wear? If your condition affects your walking, then please bring a pair of shorts or wear loose fitting clothing to allow you to be assessed more thoroughly. For a shoulder or arm assessment, ladies may prefer to wear a thin strapped top.

Can I bring a family member?  You are welcome to bring a family member, carer or friend to the appointment.  Please let us know ahead of your appointment if you will require an interpreter and we will arrange this with you.

How can I get to my appointment? There are free car parks at all of our sites including disabled bays. If eligible, you can access Patient Transport (Derbyshire : East Midlands Ambulance Service)

What happens if I am unable to make my appointment?  Please let us know as soon as possible if you cannot keep an appointment so that we can offer it to someone else.  Please contact our service and re-book if appropriate.

If you do not attend an appointment without cancelling, you will be discharged from the service after 2 weeks if we have not heard from you.


If you fail to attend consecutive appointments, or regularly need to cancel appointments we will discuss with you the practicalities of future appointments.

Please make us aware if your situation changes and you no longer require treatment.

Students:  Sometimes we have students in the department working closely with our therapists.  If you would prefer not to be treated by a student please let us know in advance.
