What is the DCHS Chaplaincy service?

Derbyshire Community Health Services NHS Foundation Trust and Chaplaincy Derbyshire are working together with your community.

Sometimes life can be challenging and bring up worries, anxieties and questions. We offer support, comfort and a confidential listening ear as part of our community healthcare team.

We are a Christian pastoral service, but are here for people of all faiths and none.

If you would like to meet your community chaplain please speak to your GP surgery team or contact the patient experience team on 01773 525 119.

Spiritual wellbeing form

  • as a reminder that all staff, whatever their discipline, can and do provide spiritual care in the course of their normal duties
  • to provide a clear place for all staff to document the assessment and care planning for spiritual needs, if they have not been noted elsewhere.
