About us
Our community therapy services are provided in the home and other community settings for vulnerable, elderly or frail people and their carers. Our community therapy services work in partnership with Derbyshire Adult Social Services and other statutory and voluntary agencies.
We provide physiotherapy and occupational therapy assessment, treatment and advice for individuals in their own home, working jointly with patients and their carers to establish goals and priorities.
Our service looks after adults and older adults across Derbyshire.
The service is available to individuals who are unable to access an outpatient department or where the home environment needs to be considered as part of the assessment and treatment process.
Community therapy services are provided across Derbyshire in patients’ homes and community residential settings with bases at the following locations;
