Post-COVID Service
The Post-COVID service sees people in Derbyshire who have post-COVID symptoms.
The service is provided by a multi-disciplinary team including GPs, ACPs, Health Psychologists, Physiotherapists, Occupational Therapists, and support staff and is delivered by Derbyshire Community Health Services.
Access to the service is via a GP referral only.
What does the Post-COVID service offer?
The Post-COVID service supports the rehabilitation of people struggling with ongoing symptoms of COVID-19. The aim of the service is to educate and support people to understand and self-manage their condition better. The post-COVID service offers assessment and investigation followed by 1:1 and/or group rehabilitation. The group rehabilitation offer includes a virtual education programme covering common symptoms such as fatigue, breathlessness, and cognitive issues. The service may refer to other specialist services where clinically indicated.
How to refer to the service?

Please note: When referring people to the service, please ensure that pre-existing medical conditions are optimised and relevant abnormal tests investigated prior to referral. This is to ensure that other potential causes of symptoms have been ruled out and that residual symptoms are appropriate for management within the post-COVID service.
