Our service
The Community Heart Failure Service gives support and treatment for patients diagnosed with heart failure. Heart failure is when the heart cannot move blood around the body as well as it would like. We are a team of nurses that give care in the Derbyshire area split into north and south teams.
What we do
We help lower the chance of you going into hospital and try to make you feel better. We do this through giving information, changing tablets and telling other health services you need help – this may be the GP for example. We may call you over the phone, see you in a clinic or see you in your own home (if you cannot leave your house).
You will be referred to us:
- By your GP
- After coming out of hospital
- By other health specialists
- If you have been seen by us in the last 12 months you can call us to be seen again.
