What is Cardiac Rehab?
A programme of exercise and information offered to anyone living with a heart condition.
- After a heart attack
- Bypass, valve surgery or stents
- Diagnosis of heart failure
- Angina
- Pacemaker or Defibrillator fitted

Programme Information
Following your individual assessment you will be offered a free nine-week programme which includes eight exercise classes and one education session.
Exercise classes run weekly at local leisure centres across North Derbyshire with home exercise advice given between. Alternatively, a home exercise programme with weekly telephone support can be provided. Our team will adapt exercises to suit your individual needs if you have other long-term conditions such as osteoarthritis or breathing problems.
All patients, relatives and carers are welcome to attend our education sessions. An online platform called RECAP health; is another way your cardiac team can share helpful information with you on an individual basis.
If you would like to be referred to this service your health practitioner or GP can fill out a referral form and fax, email or post it to the team.

For all patients who complete a Cardiac Rehabilitation programme we refer directly onto an appropriate maintenance exercise programme in their local area.
The British Heart Foundation website is very helpful for patients, carers and other health care professionals. All resources can be downloaded or ordered for free.
Active Derbyshire is a county wide campaign to encourage and support people to become more active by providing you with the information of what activities, classes and clubs are in your area.
