JUCD Integrated Care Strategy
The Joined Up Care Derbyshire (JUCD) Integrated Care Strategy has been approved and sets out how JUCD will improve the health of the people of Derby and Derbyshire.
The strategy outlines 3 key areas of focus, 'start well', 'stay well', 'age/die well' and JUCD are holding a series of online events to talk about what these areas of focus mean and how we hope to achieve the aims set out in the strategy to improve the health of our population.
There will be a day event and an evening event, the evening event will have a BSL interpreter present.
Online Events
Start Well - To improve outcomes and reduce inequalities in health, social, emotional, and physical development of children in the early years (0-5) via school readiness. (11 May, 18 May)
Stay Well - To improve prevention and early intervention of the 3 main clinical causes of ill health and early death in the JUCD population - circulatory disease, respiratory disease and cancer. (16 May, 22 May)
Age/Die Well - To enable older people to live healthy, independent lives at their normal place of residence for as long as possible. Integrated and strength based services will prioritise health and wellbeing, help people in a crisis to remain at home where possible, and maximize a return to independence following escalations. (10 May, 18 May)
