Datix
Datix is an online system for all staff to report any incidents and risks. As an organisation we're keen to learn from the experiences of our patients and staff. We encourage staff to report all risks, incidents or near misses.
The incident reporting policy states that incidents should be reported within 24 hours or towards the end of your working shift. The Incident Manager then has a further 48 hours to review and approve the incident.

DCHS staff: You need to log in to MyDCHS (via link at top of the page) to access further information and links to add a Datix incident and or risk.
Sometimes if we are involved in or witness an incident it can cause feelings of distress and we may need to seek some support. Talking to your line manager about how you are feeling can be very helpful, however you may wish to speak to someone independent. You can access counselling via Resolve by calling 01246 525951 or at resolve@nhs.net.
If you need to speak to someone urgently and are a Derbyshire resident, you can contact the Derbyshire Mental Health Helpline and Support Service on 0800 028 0077. For non-Derbyshire residents you can find your local support service at Mental Health Helpline for Urgent Help - NHS (www.nhs.uk) and of course the Samaritans helpline is available to anyone at anytime on 116123.
